First Visit

All new patient forms are found on this website under patient registration. Please download the documents and bring them with you to your first visit. Please plan to arrive 15-20 minutes prior to you visit so that we may begin your visit on time

Your initial appointment with us will consist of a thorough, comprehensive examination, review of medical history, and a set of x-rays if you have not had them in the last 3-5 years. We care about you, your overall health as well as your dental health!

Please assist us by providing the following information when you schedule consultation or new patient visit:

  • When x-rays were taken by a previous dentist, and ask for them to be sent by email to our office.
  • A list of medications you are presently taking.
  • If you have dental benefits, please be prepared to give us the following: ID #, Group #, Employer information.  This will save time and allow us to verify eligibility of coverage prior to your visit.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your  previous dentist  has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken in our office.
